Referral Information

To Parents & Patients Needing a Referral
(Important Information - Please Read Carefully)

WHAT IS A REFERRAL?

A referral is the process by which we submit medical information to a patient's health plan and receive preauthorization for the patient to receive non-primary care services, such as those of a specialist, hospital or medical imaging facility. Many health plans require preauthorization to ensure payment for covered benefits.

WHY DO I NEED A REFERRAL?

Many health plans require prior approval by both the primary care provider and the health plan in order for your non-primary care services to be covered. If the referral has not been approved prior to the visit, specialists will often require payment by the parent/patient in rder for the patient to be seen.

HOW DO I GET A REFERRAL? Fill out a referral form

Your provider makes a referral.

We contact your health plan and request preauthorization
- Approval may take 3-5 working days.
- Referrals received after 3:00 P.M. will be processed the next business day.
- We contact you with your preauthorization information.
- You make an appointment with the approved specialist.

Please notify us prior to your first visit if you or the specialist have not received the preauthorization information.
